The undersigned agrees that Future Prospects Management, its employees, and agents will not be held responsible for any accidents or loss, however, caused, and hereby releases each of them from all claims and damages, which may arise as a result or by reason of such accident or loss.
Given that Future Prospects Management may take pictures/videos of registered players during program/activities, I authorize Future Prospects Management to take such pictures/videos of my child and understand that they may be used for future promotional purposes without any compensation. Such material will remain the property of Future Prospects Management. I consent to receive commercial electronic messages from Future Prospects Management. It is understood that I accept the above terms and conditions with this registration.
